Born in the US:

1. A completed SS-5 form 
2. Proof of Citizenship - A Birth Certificate or Passport
3. Legal Name Change - Marriage Document, Divorce Decree, Court Order Name Change
4. Identity - One identity document in your old name ( can be expired) and one identity document in your new name but this one must be current. - U.S. Driver's License, State Issued Nondriver ID card, Passport
5. You may be able to use - Employee ID Card, School ID, Health Insurance Card as long as it is not a medicaid card, US Military ID, or an Adoption decree if you are unable to produce one of the articles in the identity section within 10 days.

Foreign Born US Citizen - Adult  

1. A completed SS-5 form 
2. Proof of Citizenship - U.S. Consular Report of Birth, US Passport, Certificate of Naturalization, Certificate of Citizenship.
3. Legal Name Change - Marriage Document, Divorce Decree, Court Order Name Change, Certificate of Naturalization
4. Identity - One identity document in your old name ( can be expired) and one identity document in your new name but this one must be current. - U.S. Driver's License, State Issued Nondriver ID card, Passport
5. You may be able to use - Employee ID Card, School ID, Health Insurance Card as long as it is not a medicaid card, US Military ID, or an Adoption decree if you are unable to produce one of the articles in the identity section within 10 days. 

 Non-Citizen

Take or mail this information to your local Social Security Office. Find an office near you.

Your social security number will remain the same. All documents need to be originals.

Change your name on your drivers license in Washington:

1. $10.00 Fee
2. One of the following primary documents:
      Marriage Certificate or License
      Court Order
      Divorce Decree
      Original Birth Certificate
      Other Legal or Court Document

Take the information to your local Drivers Licensing Office.

Change your name on your Drivers License in Oregon:

1. Visit a DMV Office or make an appointment
2. Complete a DL 44 (Must be an original)
3. Give a thumb print
4. Have your picture taken
5. Pay the fee of $21.00

You will need to bring Birth Date or Legal Presence Documentation

• Valid foreign passport with a valid Record of Arrival/Departure (form I-94)
• Certification from California Department of Corrections or California Youth Authority
• Employment Authorization Card
• Permanent Resident Re-entry Permit
• Refugee travel document
• "Processed for I-551" stamped in a valid foreign passport
• Valid I-94 stamped "Refugee," "Parole or Parolee," "Asylee," or Section 207, Section 208, Section 209, Section 212d(2), HP or PIP
• Immigration judge's order granting asylum
• Certified court order or judgment issued from a court of competent jurisdiction.
• Valid I-94 with attached photo stamped "Processed for I-551 temporary evidence of lawful admission for permanent residence"
• Notice of Action (I-797 Approved Petition)
• Mexican Border Crossing Card with valid I-94

or a True Full Name Document

• Adoption documents that contain the legal name as a result of the adoption.
• A name change document that contains the legal name both before and after the name change.
• Marriage certificate (issued from a local or state Office of Vital Statistics).
• A certificate, declaration, or registration document verifying the formation of a domestic partnership.
• Dissolution of marriage document that contains the legal name as a result of the court action.
• A completed Medical Information Authorization form (DL 328) in conjunction with a gender change.

This document must be issued by a government agency within the United States or a foreign jurisdiction that is authorized to issue such documents. The document must be a legible and unaltered original or certified copy with a government seal, stamp or other official imprint. You will need to surrender your current driver license and/or ID card.
